I clicked on ‘Request
Password,’ filled out the Web profile form, and received a message that I have
already been issued a login name and password. What are my next steps?
Contact Gloria Miskevics at 630/954-1283 or Miskevics@uhc.edu to evaluate your Web account. You can
also click on ‘Forgot My Login/Password’ if you have selected Request Password
in error.
I clicked on ‘Request Password,’ filled out the
Web profile form, but have not yet received an e-mail from UHC with my login
and password.
If you are from a UHC member institution and provided the
appropriate business telephone number and business e-mail address (non-Yahoo,
Hotmail, AOL, etc.), the login and password generation process may take up to 1
hour to complete (pending activity levels). If you do not receive an e-mail
from UHC within 1 hour of requesting a login and password, contact Gloria Miskevics at
630/954-1283 or Miskevics@uhc.edu.
If you provided a personal e-mail address, or selected “Other” as
your organization, you should receive an e-mail from UHC within 1 business day.
If you do not receive an e-mail from UHC after 1 business day, contact
Gloria Miskevics at 630/954-1283 or Miskevics@uhc.edu.
I received a login and
password to the UHC Web site, but can’t access certain Web pages or databases
from the UHC Web site.
Subscription requests such as Clinical Data Base, Patient
Safety Net, and Nurse Residency Program may require
additional participation fees, licenses, or additional security rights to
access that area of the UHC Web site. Verification must be performed and UHC
staff must approve the request before granting access. This process may take up
to 5 business days. If you can’t access the Patient Safety Net, click here.
When requesting a password, I do not see my
institution in the ‘Organization’ drop-down list.
Only those institutions that have paid UHC membership fees will
appear in the Organization drop-down list. If your institution is a clinical
component of a listed organization, you may select that master Organization. If
you do not see your institution, contact Webmaster at webmaster@uhc.edu.

Existing Users
I am typing my login and
password correctly, but I keep getting a message that it is not the correct
login and/or password.
Because of UHC Password Policy, passwords are case-sensitive.
Passwords are required to be in their proper case. If your password is
“HospitalDocs2003,” this means that the “H” and “D” must be in upper case, and
match exactly.
According to UHC Password Policy, you are only allowed 5 chances to log
in. If you fail on your fifth attempt, the UHC Web site locks you out. You can
then use the Forgot My
Login/Password process to have your password reset and sent to
your business e-mail address.
How many login attempts does it take before I’m locked out of the site?
According to UHC Password Policy, you are only allowed 5 chances to
log in. If you fail on your fifth attempt, the UHC Web site locks you out. You
can then use the Forgot
My Login/Password process to have your password reset and sent
to your business e-mail address.
If I am locked out of the Web site, can I get a reset password sent to
me?
Yes! Our simple Forgot My Login/Password function
unlocks your account. Simply follow the instructions and you will receive a
newly reset password via e-mail within minutes.
I forgot my login name and password to the UHC Web
site. Can I have that information sent to me?
Absolutely! Use Forgot My Login/Password if you have misplaced or
forgotten your web account information. An e-mail will be sent to you within 1
hour containing your login name and a temporary link to set your password.

Patient Safety Net Users
I requested a UHC login and password, but I still
do not have access to the Patient Safety Net. I am getting an access denied
message. What does this mean?
The Patient Safety Net requires a second layer of security before
you can access the online tool. Only a UHC PSN representative can apply the
second layer of security. You can contact a PSN representative at psninfo@uhc.edu.
My institution participates in the Patient Safety Net, but I
don’t know who my on-site administrator is to obtain access. How can I find out
that information?
